Community Service Award Information
The Board of Trustees annually recognizes and honors those students who during their senior year in high school provide service to non-profit organizations. This
recognition will take place as a part of the Senior Awards ceremony at each school and the student may wear the medal at the graduation ceremony.
1. Between June 4, 2016 and May 1, 2017 the student must complete a minimum of 80 hours of voluntary service to a non-profit community
organization(s). (Time spent competing in events or attending conferences/events for your high school club does not count towards this total.)
2. Students must not receive any other recognition/award, school credit and/or pay for this voluntary service.
3. The organization and activities must be described below and the student must obtain a verification signature and phone number of the supervising
adult. You are welcome to attach any supporting documentation.

Community Service Form-2017 make sure this form is submitted to the College and Career Center at Fremont High School no later than May 1, 2017
