Jesus Ramirez coordinates services, provides parent-to-parent support for families and serves as a communications liaison between families, community agencies and the school, including Sunnyvale Community Services, Columbia Neighborhood Center, Mayview Community Health Center and the YWCA.

Wellness Check-In Form
You can request to speak with a School-Based Therapist or School Psychologist by filling out our FUHSD Wellness Check-In Form. We take appointments during regular school hours, Monday-Friday from 8 a.m. to 3:30 p.m.
If this is a medical or mental health emergency, DO NOT fill out this form. Call 911 or visit your nearest hospital emergency room if you can get there safely.
FUHSD SCHOOL LINKED SERVICES REFERRAL FORM
Fremont High School staff can refer a student to the District's School Linked Services Specialist for referrals to community resources by filling out this form.
